​How to File A Complaint

If you were denied language access to a federally funded program or activity, you may contact the federal agency funding the program (e.g. Department of Health and Human Service for Food Stamps. If you wish to make a complaint, you can contact the New York State Office of the Attorney General, the U.S. Department of Justice or the New York State Division of Human Rights.

Suffolk County Human Rights Commission is mandated to take complaints regarding the Suffolk Police Department. If you wish for file a complaint with the Suffolk County Human Rights Commission, please fill out the forms posted below. The forms are available in English and Spanish.
